Recipient Rights

All recipients have the right:
  • To services suited to their condition, provided in the least restrictive setting which is appropriate and available.
  • To a safe, sanitary, humane treatment environment.
  • To be free from abuse and/or neglect.
  • To an individualized treatment plan, developed through a person centered process.

Complaints:

  • ALL consumers have the right to file a complaint without fear of retaliation.
  • If a Provider/Home staff is given a complaint by a resident, it must be forwarded to the BHPi ORR immediately. (By fax or mail)
  • Providers must have complaint forms available for residents.
  • Residents can call the BHPi ORR anytime they wish to file a complaint.

Incident Reports:

  • Incident Reports document any unusual events.
  • Incidents include anything that disrupts the normal routine or program.
  • All staff involved in or witnessing the incident must fill out an Incident Report.
  • Incident reports must be submitted within 24 hours of the incident to the BHPi Office of Recipient Rights.
